I understand if I pay after the 10th of each month, I will recieve a $25 late fee on my account. I understand the recital/production fee and registration fee is NON - REFUNDABLE. I understand that Joh'anna's Jazz in Motion is not responsible for any personal belongings left at the studio. I understand my tuition are payment of 10 installments (for a full season of dance/tumbling). I understand if the studio has to close for any reason (An act of nature) for over a long period of time, we will continue class through ZOOM virtual class! Billing options: 10 installments of tuition Tuition is a set yearly fee and divided into 10 payments for your convenience regardless of how many lessons pertain in a month based on holidays, absences or inclement weather. Tuition is due by the 10th of each month. It is your responsiblity to make tuition payments on time. You will NOT receive a monthly bill. If your account is persistently overdue, Joh'anna has the right to not allow your child into class. DO NOT interrupt any class to give a teacher payments. Costumes cost (Due by NOVEMBER) Costumes are ordered in December. The costumes range from $60 - 96.00 per costume. a 50% deposit is due in November and the balance is due in DEC. If you want to make extra payments each month, that would be fine! Recial/Production fee (DUE BY OCTOBER) Joh'anna's Jazz in Motion has a production at the end of each season so parents, grandparents and friends can see the progress of each student. Our usual performance is held at the Lafon Center. Our production is usually the 3rd full weekend of May . The production/ recital fee is NON - REFUNDABLE. This fee includes, one trophy or plaque for each student, a recital program, a recital t-shirt, and 4 tickets to the recital per family. Extra tickets can be purchased through the Lafon Center. Recital / Production Fee: $125 for one child $175 for two children $250 for 3 or more children What classes are you interested in? Parent Signature As the parent of _______________________, I have chosen to enroll my child at Joh'anna's Jazz in Motion for dance/tumbling instruction. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities. I understand that Joh'anna Walker-Hebert, Joh'anna's Jazz in Motion or the instructors is NOT in any way responsible for such determinations. I understand she will attend the auditions August 1st (Cost $10/child) registration Fee $50 The registration fee is $50 per student and is NON -REFUNFDABLE. This fee is to reserve your spot at our studio. Non Sufficient Funds Check All NSF checks will result in a $25.00 fee. This fee and the NSF check must be paid in full 15 days after received to the studio . If not paid in full, you will be reported to the Parish District Attorney for collections. Private lessons We offer private lessons for students that would like to have the one on one experience with a teacher to improve their technique. This cost is $45 for 45 minutes for one student. All Private lessons MUST go through Joh'anna, regardless of the teacher that will be giving the private. Thank you!
